說明：
(一) 報名資格：凡年滿12至24歲 (出生年為民國87年至99年間)之青少年。
(二) 報名方式：111年1月28日(星期五)前，備妥報名資料寄至「國立虎尾高級農工職業學校學務處」(632004雲林縣虎尾鎮博愛路65號)，信封上請註明「教育部國民及學前教育署第四屆青少年諮詢會之青少年代表報名」（郵戳為憑）。
(三) 遴選方式：第一階段資格初審、第二階段書面複審及第三階段面試決審。
